丹里"}],"nameIdentifiers":[{}]}]},"item_20002_date_19":{"attribute_name":"日付","attribute_value_mlt":[{"subitem_date_issued_datetime":"2019-05-13","subitem_date_issued_type":"Created"}]},"item_20002_description_14":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"家族性地中海熱は遺伝性自己炎症性疾患の一つで、12～72時間持続する38℃以上の発熱と無菌性腹膜炎、胸膜炎、関節炎などの漿膜炎症状を反復する。本邦ではまれな疾患と考えられていたが、責任遺伝子であるMEFV遺伝子が同定されて遺伝子検査が可能となり診断例が増加している。女性では月経が発作の誘発因子となるため、月経期の発作は子宮内膜症に類似した症状を呈する。当科で最初に経験した症例は、月経期に38℃台の発熱、腹部全体の疼痛を反復していた。子宮内膜症性嚢胞を合併していたためジエノゲストを開始したところ、発熱発作は消失した。子宮内膜症による症状であったと考え、挙児希望のためジエノゲストを中止し腹腔鏡下手術を施行した。しかしその後も発熱が持続したことから内科医が本症を疑い、遺伝子検査および診断的治療とされるコルヒチンの有効性から本症と診断された。当初は希少例と考えていたが、本症を認識して診療にあたったところ、月経期の発熱を契機に診断された症例を5年間で8例経験した。すべての症例にMEFV遺伝子変異がみられ、コルヒチンが有効であった。8例中5例に子宮内膜症の合併がみられたが、興味深いことにジエノゲストは投与された4例全例でコルヒチンと同等かそれ以上の発作抑制効果を示した。また、8例中3例で発症後の妊娠がみられたが、妊娠期間中は症状が消失した。家族性地中海熱は産婦人科通院患者の中にも相当数含まれる可能性があり、明らかな感染源がないにもかかわらず月経時に38度以上の発熱を反復し、腹痛および胸痛を伴う場合には本症をある程度疑う必要があるものと考えられた。","subitem_description_type":"Other"}]},"item_20002_description_20":{"attribute_name":"資源タイプ","attribute_value_mlt":[{"subitem_description":"Article","subitem_description_type":"Other"}]},"item_20002_description_22":{"attribute_name":"その他の資源識別子","attribute_value_mlt":[{"subitem_description":"日本女性医学学会雑誌.
