ClassⅡすなわち成人骨格性上顎前突症患者における矯正治療前後の顎関節形態変化について顎関節断層規格X 線写真を用いて検討を行った。【資料および方法】松本歯科大学病院矯正歯科で、骨格性上顎前突症と診断され、小臼歯抜歯後、外科的矯正治療法を用いずマルチブラケット装置単独で治療した不正咬合患者19 名（女性19名）を対象とした。なお、下顎骨偏位、顎関節症状、下顎頭や下顎骨変形を示す症例は含まれていない。動的治療開始時の平均年齢は24.3 歳で、平均動的治療期間は2 年7か月であった。初診時、動的治療終了時および保定開始から2 年後に撮影した顎関節断層規格X 線写真と、同時期の頭部X 線規格写真で顎顔面形態変化を解析し、顎関節形態変化と顎顔面形態変化との関連性を検討した。【結果および考察】顎顔面形態の変化は、動的治療終了時にU1-FH とoverjet は有意に減少し、interincisal angle は有意な増加が認められたことから、骨格性上顎前突症の動的矯正治療は目的通りに行われた。また、保定開始から2 年後にU1-FH、overbite がわずかに有意に増加し、interincisal angle はわずかに有意に減少したものの、良好な咬合が維持されていた。顎関節形態の治療後の変化は、関節結節後方斜面傾斜角および下顎窩の深さは初診時から動的治療終了時および保定開始から2 年後に有意な増加が認められ、下顎窩の前後的長さは初診時から動的治療終了時および保定開始から2 年後に有意な減少が認められた。一方、下顎頭および顎関節腔の形態は、初診時から動的治療終了時および保定開始から2 年後に有意な変化は認められなかった。正常な顎機能を有する者では、上顎前突者に対して矢状顆路角より矢状切歯路角が急勾配であることが報告され、さらに、上顎前突者には下顎切歯点の咀嚼運動経路において前方型シフト開口が報告されていることから、本研究でみられた関節結節後方斜面角、下顎頭滑走斜面角の増加は、成人骨格性上顎前突症患者では前歯部被蓋の改善によるアンテリアガイダンスの付与により、咀嚼運動時の下顎骨の前方運動が減少し、そして、下顎窩への負荷が減少することで、顎関節の形態的適応変化が生じる可能性が推察された。【結論】成人骨格性上顎前突症患者では小臼歯抜去による矯正治療で、上顎前歯舌側傾斜による正常な前歯部被蓋の改善に伴い、関節結節後方斜面傾斜角が急勾配になり、結果として下顎窩の深さが増加した。すなわち、矯正治療による咬合の改善に伴い、下顎頭が機能運動する下顎窩の適応現象が生じることが示唆された。","subitem_description_type":"Other"},{"subitem_description":"2014","subitem_description_type":"Other"}]},"item_20003_description_21":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"application/pdf","subitem_description_type":"Other"}]},"item_20003_description_22":{"attribute_name":"その他の資源識別子","attribute_value_mlt":[{"subitem_description":"甲第163号","subitem_description_type":"Other"}]},"item_20003_dissertation_number_55":{"attribute_name":"学位授与番号","attribute_value_mlt":[{"subitem_dissertationnumber":"甲第163号"}]},"item_20003_identifier_23":{"attribute_name":"資源識別子URI","attribute_value_mlt":[{"subitem_identifier_type":"URI","subitem_identifier_uri":"http://id.nii.ac.jp/1070/00002399/"}]},"item_20003_version_type_51":{"attribute_name":"著者版フラグ","attribute_value_mlt":[{"subitem_version_resource":"http://purl.org/coar/version/c_970fb48d4fbd8a85","subitem_version_type":"VoR"}]},"item_access_right":{"attribute_name":"アクセス権","attribute_value_mlt":[{"subitem_access_right":"metadata
