We encountered a patient who required implant removal due to poor oral hygiene. The patient, a 67-year-old man, had been fitted with dental implant in September, 1985. He had shown good oral hygiene for 6 years after implantation, but during the next 2 years he did not attend his scheduled checkups. It was during this time that the poor oral hygienic condition resulted in persistent pus discharge and swelling around the peri-implant gingiva. X-ray examination disclosed a bone defect around the implant.
